The fact that chemical accidents continue to happen that have great similarity to past events indicates that the necessary learning is not taking place. The thematic session looked at why stakeholders are not learning from accidents; what are some of the fundamental organisational root causes of accidents and barriers to organisational learning; and recommendations for improvement. Why are we not learning from chemical accidents? It is frequently observed that similar accidents keep repeating
